Cruise itineraries in the eastern Aegean list one or the other, and occasionally both. They are not interchangeable, and the difference is not the ports — it is what an hour and a half of coach does to a seven-hour day.
The numbers

Both figures are one way, in normal traffic. Add fifteen minutes to the İzmir number on a summer Friday afternoon.
From Kuşadası
The berth is in the middle of the town, so you walk off the ship and into the taxi rank. Two hours of overhead on a nine-hour call leaves six or seven hours ashore, which is Ephesus properly plus Selçuk plus lunch. The arithmetic is here.
This is the port the region is set up for. Almost every Ephesus excursion sold anywhere departs from it.
From İzmir
Alsancak is a working terminal in a city of four million. The drive south takes you down the D550 past Torbalı and Selçuk.
On a nine-hour İzmir call the arithmetic works: three hours of transfer and margin, three hours at Ephesus, and time for the museum in Selçuk on the way back. On a six-hour call it does not — you would spend half your day ashore looking at the motorway.
What İzmir gives you instead is the city: Konak Square and the clock tower, the Kordon waterfront, the Kemeraltı bazaar, and the Agora of Smyrna, all within walking distance of the terminal. If your call is short, that is a better day than a rushed Ephesus.
If both are on your itinerary
Do Ephesus from Kuşadası and İzmir from İzmir. The temptation is to see Ephesus twice over — do not; the second visit will be the same walk in a hotter part of the day.
What else each port reaches
From Kuşadası — Ephesus, Selçuk and its museum, the House of the Virgin Mary, Şirince, and the beach at Pamucak.
From İzmir — all of the above at the cost of an extra hour and a half each way, plus two things that are genuinely closer from İzmir than from Kuşadası: Pergamon at Bergama, about two hours north, and Çeşme and Alaçatı out on the peninsula.

Is Ephesus closer to İzmir or Kuşadası?
Kuşadası, by a long way: about 18 km against 80. The drive is 25 minutes from Kuşadası and around 75 from the İzmir cruise terminal.
Can you visit Ephesus from an İzmir cruise call?
Yes, on a call of eight hours or more. On a shorter stop the three hours of driving eat the day, and İzmir itself is the better use of it.

For Ephesus, Kuşadası. For a city day — Konak, the Kordon, Kemeraltı — İzmir. Ships call where they call, so the useful question is how many hours you have.
